So many thing are wrong here

Replace 

if (isset ( $_POST ['action'] ) && isset ( $_POST ['action'] ) == 'Log In') {

With 

if (isset ( $_POST ['action'] ) &&  $_POST ['action'] == 'Log In') {

Too many things to replace .. hold on while i rewrite the script for you
