It will indeed only refresh the current project (or, more specifically, the current selection in the project explorer). I just click somewhere in the project explorer, do Ctrl+A to select all projects and then press F5 to achieve a complete refresh.

In testing here, Firefox and IE do not but Chrome does send the referrer (though this is inconsistent as well), regardless of whether it’s going to the same domain or not. Perhaps you want to check Preferences > General > Workspaces > Refresh using native hooks or polling. You can see that the eclipse refreshes the file you edit with external editor (like vi, emacs, etc) when you check the option.
